• High Dosage Tutoring - Support Form

  • This form is intended for use by users of our High Dosage Tutoring service who are having technical or administrative issues.  Please fill in all necessary information below.  One of our Technicians will respond ASAP.

  • NOTE: Only choose this category if you are unable to attend a session that is taking place WITHIN THE NEXT 2 HOURS due to an unexpected emergency. For non-urgent substitutions, use the "Request Substitute" button in LEO.

    ** Per policy, all substitute requests must be made with at least 24-hour notice within LEO. A tutor must be present for 95% of their tutorials or they risk removal. Adherence to our attendance policies is required for future staffing opportunities.

  • NOTE: Only choose this category if your students WILL NOT ATTEND the next upcoming class session (due to a holiday, assembly, field trip, etc.)

    ** Please include date and time of the session, the reason the class will not attend, and the date and time you'd prefer to schedule a make-up session **
